- Missing since 1-24-62 at least (perhaps already missing at the time Boak published the P.Mich. V). Upper edge badly damaged; broken on the right side. Col. II-III of verso are so blurred as to render their reconstruction almost hopeless

- The writing is much rubbed and blurred, but is almost certainly in the hand of Kronion. (Boak)
