¶ This Record must be written in Parchment, and would be better in Latin, according to the Precedent ensuing.

MEmorandum, That upon the tenth* 1.1 day of Iune in the 31. year of the Reign of our Sovereign Lord Charles the Second, (by the grace of God) of England; Scotland, France and Ireland King, Defender of the Faith, &c. it was (according to the form of an Act of Parliament in that behalf lately made, intituled, An Act to prevent and suppress Seditious Conventicles) duly proved to

B. being of the age of sixteen years and upwards, and a Subject of this Realm, was (upon the first day of Iune now last past, in the Parish of, &c.) present at T. in the said County, under a 1.2 of exercise of Religion, in other manner than is allowed by the b 1.3 of the Church of England, (contrary to the Act aforesaid.) Of all which we have the day and year first above written made this Record under our hands and seals.
