13 Submit a signed, typewritten original of the application, including the Checklist, and six signed, exact, single-sided photocopies, in one package to: Chief, Program and Project Review Branch Extramural Activities National Institute of Allergy and Infectious Diseases Westwood Bldg. Room 703 NIAID, NIH Bethesda, MD 20892 2. TO ASSURE THE IDENTIFICATION OF YOUR APPLICATION WITH THIS RFA: a. the application form should have "NATIONAL COOPERATIVE VACCINE DEVELOPMENT GROUPS FOR THE ACQUIRED IMMUNODEFICIENCY SYNDROME" (RFA 87-AI-21) typed on line 2 of the face page of the application form; and b. the packaged application should be clearly marked "NATIONAL COOPERATIVE VACCINE DEVELOPMENT GROUPS FOR AIDS" on the outside. c. The RFA label provided with the instructions must be affixed to the bottom of the face page. Failure to use this label could result in delayed processing of your application such that it may not reach the review committee in time for review. C. Organization of Application and Suggested Modifications of Form PHS-398 (Revised, 5/82) This RFA requires the submission of a single application for the proposed NATIONAL COOPERATIVE VACCINE DEVELOPMENT GROUP. Because of the potential multi-institutional nature of an NCVDG and the special requirements in this RFA, additional suggestions regarding format and some modifications seem desirable to provide a comprehensive yet readily reviewable application. 1. It is suggested that a complete Form PHS-398 be included for each of the scientific Research Projects including the Research Project of.the proposed Principal Investigator and that these be identified as "Research Projects" and be numbered consecutively. It is suggested that the title "Project Leader" ("Principal Investigator" in the case of the section describing the Research Project of the Principal Investigator) be substituted for "Principal Investigator/Program Director" and that the Abstract of Research Plan for each of the Research Projects, in additio, to describing the work proposed, provide a statement of relevance to the overall objectives of the proposed NCVDG.
