decrease in viral load. So one consideration is to try to keep
lamivudine or abacavir as part of an adefovir-containing
regimen.
DR YE NI: I think that the largest difference between the situation (stable CD4+ cell count of 300/pL) and the one in which the
CD4+ cell count has declined to 80/pL is not so much in the type
of drugs that one is going to choose for the subsequent regimen

but with the amount of time one has in which to make the decision. In the first situation, there is some time to make this decision because the CD4+ count is high. So it might be acceptable to
wait a little bit until several more new drugs are available. In the
second case, however, the change needs to be made right away
because the CD4+ cell count is at a more critically low level.
