886 Abstracts 43197-43201 12th World AIDS Conference to develop projects and preventive actions in schools all year round. Niter6i is the 10th county with the highest HIV infection rate in Brazil and epidemiological numbers have shown that 55% of them have been sexually infected during adolescence. Results: Implementation of a preventive STD/AIDS culture through a permanent commission for STD/AIDS managed by Grupo Pela Vidda - Niter6i and public educational organs (formed by 60 teacher working in 45 schools among 6 counties, and 38.000 students taking part in varied actions for STD/AIDS prevention. The project has reached 176.000 students in 122 schools from 6 counties. Lessons Learned: Preventive work in STD/AIDS among teenagers are successful when methodologies which call for their participation are used, including themes about sexuality, reproductive life and youth pregnancy. Box 2659 Apapa Lagos, Nigeria Issue: The overall objective is to educate and train Prison Officers' Wives Association (PROWA) members, Prison Staff and Prisoners on HIV/AIDS/STDs prevention and control. Project: Based on the findings of our baseline data, which revealed a low knowledge of HIV/AIDS/STDs held for the target population (i.e. PROWA, Prisoners and Prison Staff), detailed intervention programme was carried out. The main objective was to educate and train PROWA members. Prisoners and Prison Personnel on the prevention and control of HIV/AIDS/STDs. Strategies used to achieve this Objective were Seminars Workshops Participatory Techniques, distribution of IEC materials including Condoms, both Uni-lateral and Bi-lateral techniques were used throughout the period of the project. Results: (i) After education and training, the result showed a remarkable increase in knowledge of HIV/AIDS/STDs. For instance 66.6% of PROWA participants had a good knowledge as against 41.6%. (ii) Participants knew the behaviour that put them at risk of contracting HIV. (iii) The targets were able to demonstrate proper use of Condom. Lesson Learned: Increased knowledge of HIV/AIDS could be achieved if video coverage and photographs of activities carried out were allowed in the Nigeria Prison System for the purpose of playback at intervals. 43198 A new approach to involving the community in HIV/AIDs program planning: Zambian experience Masauso Nzima1, D.A. Box 32320 Lusaka 10101; 2Central Board of Health, Lusaka, Zambia Issue: Addresses a new approach to consensus building for HIV/AIDS STD program and activity design. This initiative brings together groups from various sectors, including governmental and non-governmental structures down to the community based players. This approach builds rapport among stakeholders, identifies their comparative advantages and the experience of shared programming. It precedes and informs participatory community based evaluation. Project: Program emphasis is placed on building long-term capacity for respective stakeholders to deliver HIV/AIDS services. The program provides opportunities for government agents to directly experience the process of developing and implementing interventions at the community level. The community in turn is afforded the opportunity to understand first hand, the mandates and restrictions of the formal sector. This approach has laid valuable groundwork for research leading to a potentially sound program design and effective community advocacy. Results: The result is a measurable process that has created the basis for dynamic and thus culturally responsive HIV/AIDS programming. Tangible results include the creation of a community based task force and subsequent strategic planning for program/activity design. Critical to this process is the development of concrete and accessible monitoring and evaluation tools. Instruments are currently being adapted for meaningful use by government implementing structures down to community based programs. Lessons learned: Successful, culturally appropriate multi-sectoral response to the epidemic is an enormous challenge. The process requires patience, flexibility and maximum structured participation to ensure ownership by all stakeholders. With limited but well focussed guidance local partners were able to formulate working terms of reference (guidelines) for each district task force. |43199 | Prevention and control of STD/HIV in the community/primary health care settings Ana L. G Brasilia - DF; 2Health Community Coordination, Brasilia, DF, Brazil Issue: In the last years the AIDS epidemic in Brazil has been characterized by its ruralization, fast progression in female population and low income class. There fore, reach those population with action of prevention and control of STD/HIV is one of the most important strategy. Project: In Brazil, the actions of prevention and control of STD/HIV in the community/primary health care basis have been done by the community health care workers (CHCW). There were two program carrying on their actions with the CHCW: The Community Health Care Workers Program (CHCWP) and The Family Health Program (FHP). The principal strategy of those programs are: information, education and counseling for HIV testing of all STD patients and all pregnant women; identification and referral of STD patients and their partners to the primary health care units; and home follow up of pregnant women regarding to their VDRL status. The CHCWP team is composed by one nurse and a maximum of 30 CHCW. Each CHCW follows monthly 200 to 250 families, if in rural or urban area respectively. On the present moment, in Brazil, we have 2.217 nurses and 44.532 CHCW following 6.017.339 families. The FHP team is composed by one community doctor, one nurse, two auxiliary nurses, and four CHCW. Each CHCW follows 200 families. So, on the present moment we have 969 team of FHP with means 3.876 CHCW following 775.200 families. Those programs have been expanded along the country. Result: With the actions above described we achieve to: break the chain transmission of STD/HIV; decrease the vertical transmission of HIV; manage adequately the STD cases by using the syndromic approach and eliminate the congenital syphilis as a public health problem Lessons Learned: The identification of actions for preventing and controlling STD/HIV by using the CHCW has the advantage of giving to the population the appropriate information/education in STD/HIV/AIDS respecting their social, cultural principles and their way of communication. Baigorria, Argentina Objectives: 1) To generate AIDS prevention specific focuses in youth encounter centers. 2) Training young community leaders in order to multiply the preventive dynamic, concerning their prevention agents rol. 3) Alternative projects constructions 4) To potent their ability to multiply each leader. 5) Tests results through means of the investigation and revision of their own practices. Material and Methods: Teenagers between 16 and 17 years old (8 girls and 7 boys), from different socioeconomical levels were selected belonging, all of them from a middle class High School (Brigadier Estanislao L6pez). As it was informed before these selection was carried out according to previous special conditions required by the authors. These teenagers after receiving their training were able to spread the idea to other youngers of 15 different High Schools in our Area. In 1997 ten new teenagers have been incorporated to replace the ones who finished the secondary school last year, keeping the proyect going on. Besides, this year two workshops were done for teachers and students's parents allowing this to maintain a constant training of 15 students of different schools, public and private. Each of these activities are divided in three steps: a) workshop of workshops; b) Training days; c) Evaluation plan. Results: Starting with the first fifteen trained youth, more than a hundred qualified students, with their different modalities are working as multiplier agents within their friends, people surrounding them, and now teachers are parents always taking into account human dignity and facing discrimination and prejudices. This transference information methods seem to be effective between youth in our Region. Conclusions: The strong impact that reflection and concientization workshops produces fructify quickly. The leaders carry out and projects numerous activities (small conferences, diagnosis interviews, plays, hot lines and mail lines). Teachers are being permanently consulted about their projects which permits a clear follow up and progress of those forecoming. The youth integration, the protagonism and the experiences exchanged are pleasing additional results. 43201 Partnership between NGO and GO - The experience of the State Comission on AIDS of Rio de Janeiro, Brazil Margarete Simoes Ferreira. Health Department of Rio de Janeiro, Rua Uruguai 449 BLB App202-Tijula, Rio de Janeiro/RJ 20510-060, Brazil Issues: Rio de Janeiro State is the Second of Brazil in number of AIDS's cases. Until 31/7/1997 we had 17.523 cases notified. The State Commission on AIDS is official, legalized by the Rio de Janeiro government in 1992 in order to join efforts from NGO and GO to improve politics and works based on Human Rights and to criate new strategies to defy AIDS epidemic. With the BIRD's funds the Commission's signification was absolutely essential making project's evaluation to be financed and controlling the funds spended. The discovery of anti-virals drugs brought new possibilities to public assistance organization. The claims for more prevention actions and beds on the public hospitals were replaced for more medicines and blood tests. Project: To confront the registers of the Commission's meetings on 1996 and 1997, codifing the subjects discussed, They will be classified in five points: Prevention, Clinical Assistance (including tests, ambulatories and hospitals), Medicines, Personnel Appeals for Assistance and Institutional Development (including courses, trainers, acquisition of basic equipments, rules and vaccine's rules). Results: There were 10 meetings in 1996 where Prevention appears 21 times, Assistance 13, Medicines 11, Personnel Appeals for Assistance 2 and Institutional Development 17. In 1997 there were 8 meetings with less members than in 1996. The points discussed were: Prevention, 5 times; Assistance 3; Medicines 6; Personal Appeals for Assistance 4 and Institutional Development 4.
