Results: A training workshop delivered to care providers addressing homophobia and heterosexism has been positively received by over 500 health and helping care providers in a variety of settings in Manitoba and Canada. Follow up questionnaires point to the need for continuing education on this topic, policy and practice changes, and the need for clients to self-advocate. Lessons Learned: Internalized homophobia and cultural homophobia and heterosexism negatively affect equal access to care and treatment for gays, lesbians, bisexuals and persons infected with HIV/AIDS. Individuals practicing self-advocacy skills with care providers will receive improved care and treatment, and increased feelings of self-worth and self-esteem. 60744 Prevention and living positively on HIV/AIDS among scavengers, sweepers in urban city slums Manoharan Palani. They are drugaddicts & Sex Workers, & not aware of the dreadful disease which leads to death. Throu Non Formal & Community Health Education & coaching health care & NGO staff Training prevention & Safeguard methods are teaching of self help & co-operatives. Campaign throughout Madurai District. Project: Madurai Non Formal Education Centre (MNEC) Study team observed prevention project during traditional functions pilgrims sport with exhibition stalls and counselling centre functioning Individual, Pre & Post counselling & with publication materials especially to women community. Continous financial support definitely change their life style bring up their children with brigh future to New Social developments among them. 60745 1 Prevalence of HTLV 1 and 11 antibodies in a high risk group for HIV infection in Belem, North Brazil Helena Brigido1, Ana Lucia A.L.W. Secretaria Municipal De Saude, Belem; 'Instituto Evandro Chagas, Belem, PA, Brasil Introduction and Objectives: The seroprevalence rates of HTLV 1/11 antibodies in a high-risk group are expected to be higher than in the normal population, as transmission of these agents is similar to that of HIV. In this regard, concomitant HTLV 1 and 11 infections have been reported by some authors. The present study aims at determining the seroprevalence of HTLV 1 and 11 antibodies in patients who attend the state Reference Unit for AIDS. Method: To date, we have tested serum samples from 300 individuals. For each one data on sex, age, potencial risks for infection and schooling were recorded. Results: 122 males and 178 female were enrolled, most of them (251-84%) aged 20-44 years. Heterosexuals with multiple partners prevailed (241-86.7%) over the risk categories and a significant proportion of patients were young adults attending the high school. The seropositivity rate for HTLV 1/11 was 3.7% (ELISA - Biochem) of wich 54% were confirmed by Western Blot(only HTLV 1 infection). HTLV infection is at least six times more frequent in this group than blood donors in Belem area (0.5%). It's therefore recommended to emphasizes, the measures for control and prevention of HTLV infection. 60746 AIDS specialized service in city of third world country Yara Alvaren G.A. Drumond1, N.S. Camargos B, Belo Horizonte - MG CEP 30525490; 2Secretaria Municipal Saude Dee Betim, Belo Horzonte, MG, Brazil A small city of a third world country (Betim, MG-Brazil) could rearrange resources of the local governmental Health Department with the Nacional Governmental Heath Department to create an interprofessional team to work with HIV/AIDS patients. To establish this service, specializing in the prevention and treatment of AIDS, several steps were followed: I - Promote the involvement of governmental and philanthropic organizations; II -Training an interprofessional team (physician, pharmacist, nurse, psychologist, social assistant) to deal with HIV/AIDS; III- Establishing a local pharmacy with the drugs for treatment of HIV and opportunist disease; IV - Establishing laboratories to provide diagnostic test for viral measure ments, CD4 and CD8 cells and serology. The service was organized to receive patients in any stage of the disease evolution (ambulatory, day-hospital, hospital and homecare). The integral assistance of the patients improved significantly their longevity and quality of live. Our experience demonstrate that it is possible to organize a service with a team of interprofessionals to treat HIV/AIDS patients in the cities of a third world nation once the government assumes the responsibility of the health system. 60747 0 ARPAO: Communication and participation of inmates in the fight against AIDS Gerson B. Joao Pessoa 325 90040 - 000 - Porto Alegre, Brazil Issues: Today, the inmate population of Rio Grande do Sul is around 11 thousand inmates, divided into 94 correctional facilities which can hold 7126 inmates. Thus, there is a clear overcrowding which sholl get worse. The reality of the overcrowded prisons usually does not allow the implementation of programmes aiming at the inmates health and, thus, prevention policies for STD/AIDS. The socioeconomic conditions, the infrastructure of the correctional system and the high turnover rates in the prisons in the state make this population a target for HIV infection other STDs. Project: O ARPAO, communication and Participation of Inmates in the fight against AIDS is a project carried out in partnership with the Superintendency of Prison Services, setting an active communication process about the issues involved in the prevention of STDs/AIDS in the correctional system, creating a new information channel withthe active participation of the inmates. Results: 80 correctional facilities shall be reached throuhgh the newspaper o ARPAO on HIV and other STDs transmission modes until December, 1996, creating a institutional communication channel on the right use of the condom and the safe use of injectable drugs. Lessons Learned: O ARPAO, Communication and Participation of Inmates in the fight against AIDS is characterized by a methodology which takes into account the inmates experience and knowledge about STDs/AIDS. This makes this instrument functional and significant in the life of this population getting excellent results relative to non isolation and solidarity policies. 60748 Evaluation of the AIDS surveillance system at some hospitals Maria Regina Varnieribrito. Av Protasio Alves 4403/302 Porto Alegre-R S, Brazil Sponsored by the Health Ministry (Sexual Transmitted Diseases and AIDS Coordination), we tried to evaluated the level of uncles reporting of AIDS in the City of Porto Alegre (POA), Brazil (1,286,251 inhabitants-1996), during the epidemiological year of 1995, at the Hospitals localised in POA. Nothing all of the patients were from POA, but they were from the metropolitan are a around and sometimes farther. The mainly problem was the medical register and the unfulfilled diagnosis at the Hospital's Information System. We found 920 AIDS patients or patients with AIDS diagnosis. 69.2% (637) of the patients were actually AIDS patients. 36% (311) were in the Epidemiological Surveillance System (ESS) as AIDS cases. 848 had AIDS diagnosis at their medical registers. 282 of then were actually cases and were reported cases to the ESS. We found 40 patients that were not AIDS cases but were included as cases in the ESS. We had Hospitals with 28.9% to 56.9% of do not adequate reporting of AIDS cases. Most of them had its diagnosis at 1995. There are an urging on improvement of the Surveillance System for AIDS and a great training of the physicians in AIDS diagnosis criteria and also in the epidemiological Surveillance System. [60749 Sentinel surveillance in female sex workers 1992-1997 Maria Dominguez. Fantino Ffalco #6 Santo Domingo Dominician, Republic Objectives: To monitor the HIV epidemic in female sex workers that attend public health facilities for monthly check ups. Design: Sentinel surveillance Methods: Seven sentinel posts have been established in clinics that offer STD services for female sex workers in different regions. Every year since 1992, a number of blood samples obtained for other purposes from anonymous female sex workers were analyzed by the ELISA methods. Those that tested positive and 10% of those that tested negative were validated in a specialized laboratory. Results: The HIV prevalence is growing in all the sentinel posts. The average in 1992 was 4.3% and 9.7% in 1997. Prevalence varies according to clinics. In 1997 the lowest prevalence was 4.0% and the highest 14.7% Conclusion: HIV epidemic is growing and prevalence varies according to clinics. Even though the prevalence is the highest in Dominican Republic, the epidemic is growing at a lower speed than other population groups. El Sol De La Molina, Lima 12, Peru Objectives: To compare the strategies, development and the styles of management of each of the nine National AIDS Program Directors in Peru. Design: Retrospective, descriptive study. Methods: We compare the performance of all the National AIDS Program Chiefs, analyze their programs, strategies and development of their activities related with the profession, the medical specialty, the sex, the age, the institution where he/she worked before and the previous capacitation, and also their experience in the managing a health service. Results: A total of nine national chief performances where analyze and compared, we find finding that the directors with managing experience their priorities were to fortify the administrative development of the program in all the country.
