Data on AIDS survival in Asia is scanty. Objective: To estimate the median survival time of AIDS patients from the point of diagnosis to death in an underdeveloped zone (Manipur) of a developing country (India). Design: Retrospective study. Methods: AIDS cases diagnosed in Imphal (capital of Manipur) between 1993-1995 by the State Physician Responsible for AIDS Case Management (PRAM) located in JN Hospital re studied retrospectively from the hospital records and following up the individuals within the community. Results: Out of 71 cases diagnosed, 64 (90%) had died. Extra-pulmonary Tuberculosis was the comonest (45.3%) AIDS defining disease. The median survival time of AIDS patient was found to be 3.9 months (95% CI = 5.4-9 months). The variation of survival times was observed with age, number of initial AIDS defining diseases, initial diagnosis (Extra-Pulmonary TB only/Esophageal Candidiasis only/HIV Encephalopathy only/HIV wasting Syndrome only/Others) and marital status. Conclusion: The survival of AIDS patient in Manipur is among the shortest observed in the various parts of the World. To make AIDS patients live longer in Manipur, the government is to frame and implement provision of better management facilities of HIV/AIDS patients. 60079 Sexual practices among Nairobi University freshmen Benson Subiri Obwogo. PO Box 19676 Nairobi East Africa, Kenya Issue: Campus students represented on educated cohort of the youthful population affected and infected by the AIDS scourge. Project: This AIDS survey, targetting freshmen, tested their knowledge, Attitude, Behaviour and Practice, KABP, towards AIDS, through KABP self-administered questionnaires that were re-inforced by personal interviews conducted by fellow students who were specially-trained peer educators. Topic guide questions were used. Results: All the students (100%) felt that AIDS was real on campus although the administration had done little to curb its spread. They called for more agressive interventions into the problem, including seminars (75%), decentralized counselling services (67%), less freedom (30%), more recreational facilities (80.9%), increased stipends (98%) and AIDS test before marriage (54%). Prostitution, stress and loneliness and alcohol intake were also blamed. Although 61% had had sex recently without using a condom, 81.3% and 75%, respectively, advocated for condom use and abstinence to curb the disease. Lessons Learned: That there is great need for AIDS interventions targetted at Freshmen, and, which should be fully backed by the campus administration. That these interventions should promote condom use, abstinence and mutual monogamy and trust through peer education. Lupu Hospital Colentina Bucharest, Romania Background: The report was drafted as a feedback evaluation during a period of 1 year within the day care ward for HIV/AIDS infected children at the "Colentina" Infectious Disease Hospital - Bucharest, Romania. The appraisal group consisted of 98 children, 48 of them without ARV treatment (antiretroviral), that is group 1 and another 50 being subject to ZDV monotherapy, that is group 2. The average age of these children was around 8 years old with limits between the ages of 2 and 13. As for the disease transmission way, it was exclusively non-vertical. The skin lesion assessment was first done on both groups before the cure with ARV and then periodically repeated every 3 month. The following skin lesions were observed during the study: infectious lesions:- bacterial fungal vital parasitic inflammatory lesions malignant neoplasias Feedback: All types of lesions described above were presents in both groups; as far as the group treated with ARV, a drop was noticed in the occurrence of parasitic and fungal infections; the herpetic and zoster lesions had a slower development and a scarcer occurrence amongst the children treated with ARV; though several malignant neoplasias occurred amongst the children from both groups, these did not surface at the skin level. Comments: Though the ARV monotherapy is no longer recommended nowadays, and we have stopped using it also, it is worth emphasizing the softened development of the HIV symptomatic infection at the children who received ARV treatment. Given the lack of immunity monitoring means, the skin lesions (fungal, vital) can stand as a token for the immunity drop. 60081 Women, HIV/AIDS and advocacy: Setting priorities Juanita Chesnut1, Tamara Maynard2. 120 Richmond Place, Bronx, NY; 2Katrina Haslip LawTAP New York, NY US Issues: We will discuss barriers-and how to overcome such barriers- to HIV-positive women of color advocating for themselves. Project: We will draw upon our personal experiences of how we became advocates in our communities, in particular, how we became involved with peer education and outreach through the Katrina Haslip Law Technical Assistance Program. Results: HIV-positive women of color face gender, culture, language and education barriers, but through advocacy and outreach, women who are themselves infected become their own best advocates as they know what their needs are. For the women recourse to sex has become a necessity especially along the National Highway where promiscuous truckers pay handsomely for the casual sex. They also resort to homosexuality. Migration of men to urban areas where they encounter sex with prostitutes transfer the virus to spouses and others. The AIDS outreach programme of NGO Seva Mandir supported by 300 Health Workers, 400 trained local professionals spread over in 500 villages attempts to (1) Raise multi-sectoral awareness (2) Links STD, TB with AIDS (3) Achieve gradually improvements in peoples quality of life by adopting self-help technique (4) Try to bring changes in sexual behaviour (5) Try to augment income generating assets. 46 Teams comprised of trusted Health Workers and paraprofessionals, local village people visit door to door and discuss this difficult, not yet visible issue. Contact local restaurant owners and provide First Aid boxes, condom dispensers. Motivate people to visit local treatment centres for STDs. Contact truckers, give literature. Arrange puppet shows, film shows, prayer groups, etc. The illiterate poor women try to abstain from indulging into sex but extreme degree of poverty compel to indulge again. By providing support through our integrated socio economic programmes created an environment where such sensitive issues are discussed freely. Mutual trust has been established. Have to integrate poverty alleviation by introducing home-based income generating programmes. Their participation, Rapid Spot Test method, low cost treatment affordable at village level, change in male attitudes to contraception, fertility and sexuality and grassroots voluntarism are essential for comprehensive prevention programmes particularly when AIDS are not visible. 60083 Take care psychologique of people living with HIV/AIDS at home Chritine Gonhiloagninhoun, J.K.
