TUESDAY, 21 JULY 1992 TuC 0572 TuC 0572 HEALTHY LONG-TERM POSITIVES: MEN INFECTED WITH HIV FOR MORE NOTES THAN 10 YEARS WITH CD4 COUNTS > 500 CELLS. Obiectives: To determine the time from HIV seroconversion (SC) to the development of AIDS and death in homosexual men in the San Francisco City Clinic Cohort (SFCCC) and to describe immunologic parameters of a subsample of men infected over 10 years who remain healthy. Methods: We identified 562 men with well-defined SC dates among 6704 men enrolled in the SFCCC from 1978-80. We obtained clinical data and CD4 counts at biannual visits, and ascertained AIDS and death from crossmatches with national AIDS and death registries. Using Kaplan-Meier (KM) techniques, we determined time from SC to AIDS and death. We defined healthy long-term positives (HLPs) as men HIV infected > 10 years without AIDS with CD4 counts > 500 cells. We evaluated change in CD4 count over 3 years for HLPs with CD4 counts performed at the same laboratory from 1989-91. Results: By 12.5 years, 69% of the men developed AIDS Prression to AIDS/Death (95% CI 64-74%) and 65% died (95% CI 60-70%). Of 100 115 men with >10 years of infection and no AIDS, 39% had CD4 counts > 500 in 1991. These HLPs had a me- 80 -dian change of only -11 CD4 cells/year for the 3 years 8AIDS from 1989-91; 73% of these men had never taken AZT. 60 ~60 -Conclusions: Although the cumulative proportion of men developing AIDS continues to increase up to 14 years 40 - after HIV infection, a minority of men have stable or in-, 'Death creasing CD4 counts. This represents a subgroup of HIV 20 - infected persons who may not develop disease. We are currently investigating immunologic and genetic factors 0 which may be responsible for slowed or arrested HIV 0 2 4 6 8 10 12 14 disease progression.
