You may request C.M.E. credits by
checking the appropriate box on the
Conference Registration Form. A fee of
60 Dutch Guilders applies to all requests
for C.M.E. units. Please include payment
with the registration fee.
Official Letter of Invitation
Individuals requiring an official letter of
invitation in order to obtain a visa and
permission to attend the Conference
should contact the Dutch Foundation at
the address listed on page 20 specifying
all necessary details. Note that this letter
will state that any monetary expenses incurred in connection with the Conference are the sole responsibility of the
individual.
Registration Fee Refund Policy
A full refund of the Conference registration fees will be made for written cancellations received by 2 March 1992. A full
refund less an administrative charge of
190 Dutch Guilders will be made for
written cancellations received between 2
March 1992 and 1 June 1992. No refunds will be made for any cancellations
received after 1 June 1992. No telephone
cancellations will be accepted. All cancellation letters must be submitted to
CONGREX Holland BV at the address
listed on page 19. Please note that all refunds will be sent after the Conference.

any sessions. Registrants must present
photo identification to receive registration materials. Registration will begin at

10:00 a.m. on Saturday, 18 July 1992 at
the RAI Conference Centre.
